I found it right after I posted. It can be found in the Y_UserInterfaces plugin. In the window, drag a Y_UserInterfaces control to the window, in open event of the window you just add Y_UserInterfaces.InstallToolbar. The action for this button is in the Y_UserInterfaces control you just dragged
to the window.

There's another plugin that does it too. CarbonEvents, maybe. I don't
remember.


Yes, CarbonEvents does this as well. I don't know is Y_UserInterfaces is free (CarbonEvents is $10), but my understanding is that all the documentation is in Japanese. That's a big deterrent to me.
