I don't actually understand the design. Surely there must be something more
required of the user than merely making a change so that the form is not
entirely blank. Would it be okay for the user to click the "OK" button if
they had only entered a zip code, leaving all other fields blank? That
wouldn't make much sense, but the way you have described the problem would
seem to allow for that.
I'd just leave the OK button enabled all the time, and when the user clicks
on it, check to make sure they have entered a meaningful amount of data, and
if not, throw up an alert dialog telling them what they need to do.
